If you have selected a student massage, our students are graded on the completion of your forms. To better serve you, please complete our Clinical Intake Form & Office Policies prior to your arrival. This will eliminate delays and maximize your time on the massage table.
Returning Clients are asked to sign our office policies annually as changes may occur.
